5. How to request a record
1. Complete the prescribed form (Form 2, Regulation 7) and send it to the
Information Officer.
2. Pay the prescribed request fee.
3. We respond within 30 days, extendable by a further 30 days with notice.
4. Specify the record, the form of access required, and the right you seek to
exercise or protect.
6. Grounds for refusal
Access may be refused where the Act requires or permits it, including to protect
third-party personal information, commercially confidential information, or
privileged records.
7. Remedies
If a request is refused you may apply to a court as provided in the Act, or lodge a
complaint with the Information Regulator.
